(2) A fee of $400.00 per practitioner who is registered to provide the service at the practitioner's office or location; provided, however, that a practitioner paying according to this subsection shall not be required to provide information to the local government relating to the gross receipts of the business or practitioner. The per-practitioner fee includes each person in the business who qualifies and is registered or licensed as a practitioner under the state's regulatory guidelines. (b) Practitioners of professions and occupations authorized under this section include, but are not limited to: (1) Lawyers; (2) Physicians licensed under O.C.G.A. title 43, ch. 34 (O.C.G.A. § 43-34-1 et seq.);
